Teachers Service Commission (TSC) entry or exit report popularly known as Casualty is a document that shows that a teacher has started working or ceased working in a given station.
The one page report is completed immediately a teacher starts or stops teaching in a given school. Once the report has been completed, it is urgently distributed as follows: –Â
- ORIGINAL – To the TSC Headquarters
- DUPLICATE – To the TSC County Director
- TRIPLICATE – To be retained in school file
It is important to note that this report is an accountable document which should only be completed by either the Head of Institution or in his absence the Deputy Head of Institution.Â
Reasons that can necessitate filing of an exit report include Resignation, Termination, Retirement, Absence, Desertion, Death, Sickness, Leave, Transfer among others.
If the reason for exit is leave, the head of the institution must indicate the type of leave. For transfer, the head of institution must specify the next institution.
Reporting to a station after employment, transfer, Reinstatement are among the reasons that can prompt head of an institution to complete an entry report.
